An applicant shall submit

1. Completed application form (required)
2. Transcript for All Semesters (required)
3. Other Supporting Documents

Please merge all materials into one file, name it ‘Application for Distinguished Academic Scholarship_SID_Name', and send it to the office (hytu@mx.nthu.edu.tw) within the given time period.
